Application Process & Forms

Join us at an admissions event to receive a printed application packet, or let us know if you would like us to send you an application packet in the mail! If you have any questions about the application process or about our school, please contact Rebecca, our Director of Admissions, at 503-567-9559 ext. 3 or at rebecca.leroux@trinityacademyportland.org.

Send to Trinity Academy:

  • Application for Admission - to be filled out by the parents [download]

  • Candidate's Questionnaire - to be filled out by the student by hand [download]

  • $50 non-refundable application fee

Fill out the parent portion, then give the following items to student's current school:
(The school will return the forms to Trinity Academy directly.)

After the parent/student application is turned in, please call or email Trinity Academy to:

  • Schedule the entrance exam

  • Schedule an interview
